The Importance of Immunology in Infertility Treatment
The immunological system plays a crucial role in conception and carrying a healthy fetus to term. Its dysfunction can be one of the causes of a couple’s infertility. Statistics indicate that the immunological factor of infertility is responsible for approximately 3–5% of infertility cases, while in cases of idiopathic infertility (without a known cause), repeated implantation failures of the embryo in the uterus, and recurrent early miscarriages, this percentage can be much higher – up to around 20%.
Indications for an Immunologist Consultation
If the results of all previous examinations are within normal limits, and despite ongoing treatment, we repeatedly fail to achieve a successful pregnancy for the patient, our doctor will recommend some additional examinations, which may include one with an immunologist.
Your doctor may recommend you for an immunological consultation if these basic indication criteria are met:
- Two or more spontaneous miscarriages, regardless of age
- Failure to conceive or carry a viable fetus to term after two or more embryo transfers
- In cases of confirmed autoimmune disease
Immunological Examination Process
Both partners undergo an examination with an immunologist. Immunological diagnostics focuses on a wide range of immune system problems that can affect the ability to conceive and carry a healthy baby to term:
- Presence of antibodies against sperm or the egg’s outer layer (zona pellucida)
- Impact of the Immune System on Embryo Implantation and Development
(e.g., uterine NK cells)
- Autoimmune diseases, allergies, or food intolerances
- Chronic or untreated infections and inflammations
After summarizing all completed examinations and consulting with a doctor from our center, the immunologist will establish a diagnosis and propose an individualized treatment plan. Treatment usually begins before conception and continues until a certain stage of pregnancy. Specific preparation is required in cases of repeated embryo implantation failures in the uterus.
Examination of Uterine NK Cells (uNK)
An important part of immunological diagnostics is also the examination of uterine NK cells – specialized immune system cells located in the uterine lining. These cells play a significant role in embryo implantation and placental development. Unlike NK cells in the blood, which eliminate pathogens and tumor cells, uNK cells have more of a regulatory character – they support proper embryo development and immune tolerance to the fetus.
Their excessively low or, conversely, excessive number can contribute to repeated implantation failures or miscarriages. The examination is performed by endometrial biopsy (taking a small sample of the uterine lining) at a precisely determined time of the menstrual cycle. If deviations are found, treatment can be adjusted, for example, with immunomodulatory therapy.
